Eden Care Services: Client Service Agreement


Welcome to Eden Care Services

We're dedicated to providing compassionate and reliable home health care. This

agreement explains the terms of our services. Please read it carefully and ask us any

questions you have.


Our Services

We offer a range of personalized home health aid services, which may include:

  • Personal care (bathing, dressing, grooming)

  • Medication management

  • Health monitoring (vital signs, symptom tracking)

  • Meal preparation

  • Light housekeeping

  • Companionship

  • Respite care

  • Other support services as detailed in your individual care plan.

Your care plan will be created with you (or your authorized representative) and will

specify the services we'll provide


Your Care Plan

We'll work with you to develop a personalized care plan that addresses your unique

needs and preferences. This plan will be regularly reviewed and updated as needed.


Getting Started: Information Package

Before services begin, we'll give you a comprehensive information package, including:

  • Detailed list of our services.

  • Explanation of service types and any limitations.

  • Description of how your care plan is developed.

  • A copy of this Client Service Agreement.

  • Our contact information (address, phone, email).

  • Our hours of operation and holidays.

  • Our licensing and insurance details.

  • Introduction to your Direct Care Worker(s).

  • Your service schedule (days and times).

  • Our fee schedule.

  • Contact information for a Departmental Representative regarding licensing and regulations.

  • Contact information for the local Area Agency on Aging (AAA) Ombudsman Program.

  • Information about our Direct Care Worker hiring and training.

  • Disclosure of whether workers are employees or contractors, and responsibility for taxes/obligations.

  • Your Rights & Responsibilities as a client, and our responsibilities as an agency

  • Summaries of our key policies:

    • Confidentiality and privacy

    • Complaints and compliments process

    • Gifts policy

    • Transportation policy

    • Managing client money and property

    • Scheduling and cancellations

Scheduling and Cancellations

We'll create a service schedule that works for you. Please let us know ASAP if you need to make changes.

  • Changes: Give us at least 24 hours' notice to change or reschedule a visit.

  • Cancellations: Give us at least 24 hours' notice to cancel a visit. Cancellations with less than 24 hours' notice will incur a cancellation fee of $[Amount] or [Percentage] of the scheduled visit cost.

Fees and Payment

  • Payment Methods:

    • Credit Card (Visa, Mastercard, American Express, Discover) - Contact our office to set up secure online payment or for mailing instructions.

    • Check - Mail to: Eden Care Services, 210 West Pennsylvania Ave., Suite 100, Towson, MD 21204.

    • Wire Transfer - Contact our office for bank details

    • ACH Transfer - Contact our office for bank details.

  • Making Payments: All payments are made directly to Eden Care Services. We do not accept payments to individual staff.

  • Receipts: We'll give you a receipt for each payment. Please keep these for your records.

  • Fees: Our fee schedule is available [Provide a link or explain how to obtain it]. Maryland sales tax of [Percentage] and a 3.5% processing fee will be added to your invoice. Your invoice will clearly show the total amount due.

  • No Refunds: Payments for services already provided are generally non-refundable. For pre-paid, unused services, refunds may be considered in extenuating circumstances. Contact us to discuss these situations.

  • Payment Security: We protect your payment information using [Describe security measures, e.g., encryption, secure servers]. Never share your payment details with unauthorized individuals.

Ending Services

Either you or Eden Care Services can end services with 5 days written notice. We'll discuss potential refunds for pre-paid, unused services with you on a case-by-case basis, considering the specific circumstances and your agreement
