Coordinate daily-living support after discharge

A discharge referral works best when the home-care plan is considered alongside the destination, timing, functional support needs, authorized contact, and any separate clinical requirements. Eden gives hospitals and facilities a state-specific pathway for reviewing home-care referrals without treating a face sheet, order, or requested start date as automatic acceptance.

An adult and his family member review the plan for returning home with a caregiver.

Common Transition Needs

What May Need to Be in Place at Home

Depending on the discharge plan and individual needs, support may involve:

  • Bathing, dressing, toileting, and grooming
  • Walking or transfer assistance
  • Meal preparation and meal setup
  • Medication reminders for self-administered medications
  • Household support
  • Safety supervision
  • Dementia-related routines and cueing
  • Overnight or extended-hour coverage
  • Family caregiver respite
  • Maryland private-duty nursing review based on the individual clinical needs and discharge plan

Maryland Discharge Referrals

Personal Care and Private-Duty Nursing

Maryland discharge referrals may involve non-clinical personal care or a clinical nursing pathway.

If skilled nursing is requested, Eden reviews the current orders and discharge instructions, exact nursing responsibilities, assessment information, equipment and supplies, home environment, schedule, qualified staffing, and individualized plan of care.

Pennsylvania Discharge Referrals

Non-Skilled Home Care

Pennsylvania referrals can cover non-skilled personal assistance, companionship, meals, household routines, respite, supervision and reminders within Eden’s licensed home-care scope. Skilled nursing, therapy, wound treatment and medication administration require a separately authorized provider.

What Eden Needs at the Initial Referral Stage

Start With Routing Information

The initial referral should identify:

  • Referring professional and organization
  • State, city, and ZIP where care will occur
  • Whether an authorized family member or representative should join the next-stage conversation
  • Broad requested service category
  • General schedule need
  • General expected discharge/start timeframe
  • High-level administrative context

When Clinical Documentation Is Needed

For a Maryland nursing referral, Eden may need orders, medication information, wound/device instructions, discharge records, or other clinical documentation after the initial review.

Ask Eden for the approved secure transmission method before sending clinical information. Do not paste protected records into an ordinary public website form.

Timing and Staffing

Discharge Date Is Important, but It Is Not a Staffing Guarantee

Eden can review urgent and scheduled referrals, but acceptance and start timing depend on the state, location, requested care, required assessment, staffing, schedule, documentation, and any payer or program requirements.

Ask Eden to confirm the accepted start date and staffing arrangement before relying on care for discharge.

Service Area

Confirm Coverage Before Finalizing the Plan

Maryland: Baltimore County and Harford County.

Pennsylvania: Philadelphia, Delaware, Chester, Montgomery, and Bucks Counties.

Availability still depends on the exact location, service type, staffing, and schedule.

Professional Coordination

Clear Scope Helps Avoid a Bad Transition

Eden’s role is to review the requested in-home services, establish the appropriate Maryland or Pennsylvania care pathway, and coordinate the home-care start when the case is accepted. The hospital or treating team remains responsible for its clinical discharge instructions and medical decision-making.
