Community HealthChoices and home-care questions

Community HealthChoices (CHC) is a Pennsylvania managed-care program. If CHC is part of your care arrangement, start with the name of your plan and the services being considered. Plan enrollment, service authorization and Eden’s provider participation must each be confirmed.

A program and an agency license are separate

Eden’s Pennsylvania Home Care Agency/Home Care Registry license covers non-skilled home care. It does not automatically establish approval for every public program, managed-care plan, service code, or participant.

Questions for your CHC plan and Eden

  • Which CHC plan is responsible for the request?
  • Is Eden participating with that plan for the requested service and location?
  • Which services and hours are authorized, and from what date?
  • Can Eden support the tasks and schedule currently requested?
  • Who should confirm a change in services, hours or provider before it takes effect?

Who decides eligibility?

Pennsylvania’s official program and assessment processes determine eligibility and authorization. Eden cannot promise that an applicant qualifies, that requested hours will be approved, or that a claim will be paid. Use the current state program information and the person’s authorized coordinator for those questions.

Start with a general inquiry

Tell Eden that care is needed in Pennsylvania and which program may be involved. Please do not submit insurance IDs, financial records, or medical documents through the public inquiry form. Eden will explain what information is needed through the appropriate next-stage channel.
